Effect of Recruitment Maneuvers on Oxygen Saturation, End Tidal Carbon Dioxide and Lung Mechanics in Pressure Control Ventilation Versus Volume Control Ventilation for Patient Undergoing Laparoscopic Sleeve Gastrectomy

Abstract

Background: Atelectasis occurs regularly after induction of general anesthesia in bariatric surgery, persists postoperatively, and may contribute to significant postoperative morbidity. Intraoperative recruitment maneuver improve lung ventilation, oxygenation and lung mechanics.
Objectives: The aim of this study was to compare the effects of recruitment maneuver on oxygen saturation, end tidal carbon dioxide and lung mechanics in two Groups; the volume control group and pressure control group with fixed level of PEEP.
Patient and Method: Forty patients, BMI >35 kg/ m2, who have no major obstructive or restrictive respiratory disorders where allocated in two groups (volume group and pressure group). In both groups we tested the effect of recruitment maneuver on SpO2, ETCO2 and lung mechanics (The volume group of 20 patients and the pressure group of 20 patients).
Results: We found that alveolar recruitment effectively decreased Intraoperative EtCO2 in pressure group ( P.value <0.05) while ( P.value was>0.05 ) for volume group and increase Intraoperative SpO2in BOTH groups ( P.value < 0.05) with more effect seen in pressure group. BUT, soon after extubation SpO2 decreases again and P.value was >0.05 for both groups.
Also we found that pulmonary mechanics (PIP, EXPvt) were significantly improved after recruitment (BOTH P.value < 0.05).
Conclusions: our data suggest that the use of alveolar recruitment maneuver may be an effective mode of improving intraoperative oxygenation in morbidly obese patients. Our results showed that the effect is to be short lived and end after extubation. with significant improvement in lung mechanics.

Abstract

الهدف من الدراسة: تأثير أعادة تجنيد الحويصلات الهوائية على كل من نسبة الاوكسجين في الدم وثنائي اوكسيد الكاربون في الزفير وميكانيكية الرئة في مجموعتين من المرضى الاولى بأستعمال التهوية بالضغط والثاني بأستعمال التهوية بالحجم على المرضى المصابين بداء السمنة المفرطة أثناء عمليات التنحيف بالناظور.
هدف الدراسة : دراسة سريرية تجريبية.
مكان الدراسة: الدراسه اجريت في مستشفى القديس رافاييل (الراهبات) في بغداد. الدراسة أجريت خلال عشرة أشهر من أول شهر أيار لسنة 2011 إلى نهاية شهر شباط لسنة 2012
الطريقة: الدراسة تضمنت 40مريض معامل الوزن لديهم اكثر من 35 وغير مصابين باي امراض تنفسية حادة او مزمنة و قسموا إلى مجموعتين:
المجموعة الاولى (مجموعة التهوية باستعمال الضغط): 20 مريض قسموا الى 5 ذكور و15 أناث.
المجموعة الثانيه (مجموعة التهويه باستعمال الحجم): 20 مريض قسموا الى 4 ذكور و16 أناث.
تم أخذ موافقة المرضى قبل العملية لغرض الاشتراك في الدراسة، بعد بدء العملية بأستعمال الناظور تم اعادة تجنيد الحويصلات الهوائية في كلا المجموعتين باستعمال تنهيدة الى اربع تنهيدات كل 15 دقيقة حتى نهاية العمليه او حسب قرءات مستوى ثنائي اوكسيد الكربون في الزفير بحيث لا يزيد الضغط الكلي للرئة عن 50 سنتيميتر ماء ثم بعد ذلك يكون ضغط الهواء ائناء نهاية الزفير يتراوح بين 5-8 سنتيميتر ماء الى نهاية العملية مع المحافظة على قراءات مستوى ثنائي اوكسيد الكربون في الزفير بين 40-45 مليميتر زئبق مع الحفاظ على على ضغط الرئة الكلي بمستوى منخفض. تم تسجيل قراءة نسبة الاوكسجين بالدم ونسبة ثنائي اوكسيد الكربون في الزفير كنتائج اولية للبحث وتم تسجيل قراءة كميه الهواء الخارج اثناء الزفير وضغط الرئة الكلي كنتائج ثانوية في كلا الجموعتين قبل وبعد عملية اعادة تجنيد الحويصلات الهوائية.
النتائج: عمليه اعادة تجنيد الحويصلات الهوائية كانت مؤثرة في زيادة نسبة الاوكسجين في الدم في كلا المجموعتين ولكن التأثير الايجابي لوحظ اكثر في مجموعة التهوية بأستعمال الضغط (كان مقياس نسبة الحساسيه اقل من 5 %).عملية اعادة تجنيد الحويصلات الهوائية كانت مؤثرة في خفض نسبة ثناثي اوكسيد الكربون في الزفير في مجموعة التهوية باستعمال الضغط فقط (كان مقياس نسبة الحساسية اقل من 5 %). بينما لم يكن هناك اي تأثير على مجموعة التهويه بأستعمال الحجم (كان مقياس نسبه الحساسية أكثر من 5 %).بعد اكمال العملية ورفع الانببة لايوجد اي تأثير لتجنيد الحويصلات الهوائية على نسبه الاوكسجين في الدم في كلا المجموعتين (كان مقياس نسبة الحساسية أكثر من 5 %).
الاستنتاج: النتائج تشير الى التالي :
اولاً: استعمال تجنيد الحويصلات الهوائية طريقه فعالة لتحسين نسبة الاوكسجين في الدم لمرضى السمنة المفرطة أثناء عملية التنحيف بالناظور.
ثانياً: التأثير قصير ويختفي عند القيام برفع الانببة.

Role of atropine in prevention of reflex bradycardia in response to awaked intubation in neonatal anesthesia

Abstract

Background: Neonates (0–1 months) have different anesthetic requirements. Safe anesthetic management depends on full appreciation of the physiological, anatomic, and pharmacological characteristics of this age group. One of important difference in neonates is easy development of reflex bradycardia and cardiac standstill during anesthesia. These characteristics, differentiate them from adults, and necessitate modification of anesthetic equipment, medications, and techniques.
Objectives: To evaluate the role of atropine in prevention of reflex bradycardia in neonatal anesthesia.
Patients and methods: This is a prospective study done on 30 neonate patients in Baghdad teaching hospital/Baghdad /Iraq during the period from January - July/ 2009. Neonates that underwent major surgical operations, were randomly allocated into two groups, group (A) received atropine(0.015mg/kg) 5 minutes before awake intubation and group (B) (control group) received no atropine. Heart rate was recorded at pre-induction and during intubation then anesthesia was maintained in a similar way in both groups.
Result: the results reported in this study indicate that the pre-induction with i.v. atropine was associated with a statistically significant decrease in the incidence rate of reflex bradycardia (below 100 beat/min or reduction in HR > 20% of basal heart rate) during intubation from 96.65% in the group (B) (control group) to 26.6 % in the group (A) (atropine group) In addition the magnitude of reduction in heart rate was lower by a mean of 22% in the atropine group compared to the mean reduction 41% in group (B
Conclusion: Pre-induction with IV atropine decrease in the incidence of reflex bradycardia during intubation from 96.65% in the control group (B) to 26.6% in the atropine group (A). Pre-induction of atropine reduces the magnitude of heart rate reduction during intubation by a mean of 22% compared to 41% in control group.

خلفية الدراسه: من اهم النقاط التي يختلف بها الاطفال حديثي الولاده عن كبار العمر هي سهولة حدوث انخفاض ضربات القلب الانعكاسي.
الهدف :تقييم فعالية مادة الاتروبين في منع حدوث انخفاض ضربات القلب الانعكاسي نتيجة ادخال الانبوب الهوائي الى القصبه الهوائيه اثناء التخدير في عمليات حديثي الولاده 0
المرضى والطريقه : هذه دراسه وصفيه اجريت في مستشفى بغداد التعليمي /قسم جراحة الاطفال ابتداء من شهر كانون الثاني 2009 ولغاية شهر تموز, 2009 اشتملت على 30 حاله من الاطفال حديثي الولاده من عمر يوم واحد الى عشرة ايام وبوزن يتراوح من 2.5 الى3.5 كيلو غرام .تم تقسيمهم عشوائيا الى مجموعتين .المجموعه الاولى تم قياس نبضات القلب قبل اعطاء الاتروبين ثم بعد اعطاء الاتروبين بخمس دقائق ,ثم ادخل الانبوب الهوائي وتم تسجيل النبضات . في المجموعه الاخرى تم تسجيل النبضات قبل وبعد ادخال الانبوب الهوائي ودون اعطاء مادة الاتروبين.سجلت الفروقات الحاصله في الحالتين وادخلت البيانات الاحصائيه على اساس تعريفين لانخفاض ضربات القلب(اما نزولها الى اقل من 100 ضربه بالدقيقه او نزولها الى اقل من 20% من التسجيل الاولي). النتائج: بالنسبه للمجموعه التي اعطيت الاتروبين قلت نسبة حدوث رد الفعل الانعكاسي المتمثل بانخفاض ضربات القلب الى 26.6% قياسا الى المجموعه الاخرى حيث كانت نسبة حدوثها % 96.65.
الأستنتاج: اعطاء المريض حديث الولاده مادة الاتروبين وريديا قبل الشروع بالتخدير يقلل تسبة حدوث رد الفعل الانعكاسي المتمثل بانخفاض ضربات القلب

Comparetive study between saline sonohysterography,transvaginal ultrasound and biopsy in evaluation of endometrial disease in postmenopausal bleeding

Abstract

Background : Transvaginal ultrasound is used conventionally as initial investigation of patients with postmenopausal bleeding, but saline contrast sonohysterography is a better technique to reliably distinguish focal from diffuse endometrial lesion.
Objectives: to compare the ability of transvaginal ultrasonography and saline infusion sonohysterography as initial modality for the diagnosis of endometrial abnormalities in women with PMB by correlating the results with endometrial biopsy.
Patients and methods: During the period from November 2011 to MAY 2012 ,40 female patients with postmenopausal bleeding were submitted to sequential examination by transvaginal ultrasound ,and sonohysterography .the presence of focal endometrial lesions and type of lesion (endometrial hyperplasia ,polyp, submucous myoma ,or malignancy )were noted .predictive values were calculated by correlating the results with final diagnosis reached by endometrial biopsy.
Result:The sonohysterography had 89.4 % sensitivity and 90% specificity , DA 90%, compared to 60%sensitivity and 70%specificity achieved by transvaginal sonography .
The diagnostic performance of sonohysterography for 3 main endometrial abnormalities (i.e. endometrial hyperplasia ,polyps and submucous myoma ) was better than transvaginal sonography. The best results were seen in cases of submucos myoma where sensitivity and specificity of sonohysterography reached to 100% as compared to TVS (66% and 95.24% respectively).
Conclusion:The results have substantiated that sonohysterography is a better tool than transvaginal sonography for the assessment of endometrial intra-cavity lesion .by providing accurate differentiation between focal and diffuse endometrial lesions.

Abstract

ان هذا البحث يعمد الى دراسة فعالية الفحص بالأمواج فوق الصوتيه المهبلي بعد حقن كمية من المحلول الملحي المتعادل في تقييم امراض بطانة الرحم لحالات النزف بعد سن اليأس .
المرضى والطرق: خلال الفترة مابين تشرين الثاني 2011 ولغاية شهر أيار 2012 في مستشفى بغداد التعليمي التابع لمدينة ا لطب .تم تحري 40 مريضة تعاني من النزف الرحمي بعد سن اليأس بأستخدام جهاز الأمواج فوق الصوتية المهبلي عالي التردد بعد حقن كمية من المحلول الملحي المتعادل .
لقد تم تقييم امراض بطانة الرحم الموضعية مثل (ورم ليفي رحمي تحت الغشاء المخاطي ,زوائد مخاطية وتكثر نسيجي موضعي) وتمييزها عن امراض بطانة الرحم المنتشرة.تم اجراء التحليل النسيجي بعد ذلك بواسطة جرف بطانة الرحم التشخيصي وأستئصال الرحم وحسب النتائج المستحصله تم مقارنة الخواص المتعلقه ببطانة الرحم من خلال الفحص بالأمواج فوق الصوتية المهبلي المائي مع نتائج اتحليل النسيجي.
النتائج: لقد تبين ان حساسية الفحص مقارنة بنتائج التحليل النسيجي تكون 89% والنوعية 91% والدقة التشخيصية 90%. افضل نتيجة للفحص كانت في تقييم الورم الليفي الرحمي تحت الغشاء المخاطي حيث الحساسيه والنوعية 100%.
الاستنتاجات: بقارنة نتائج الدراسة مع نتائج الدراسات الاخرى تبين ان لكل امرأة تعاني من نزف مابعد سن اليأس يعتبر الفحص بالامواج فوق الصوتية المهبلي باستخدام المحلول الملحي المتعادل الفحص الاولي والافضل في تقييم امراض بطانة الرحم وذلك لقدرة الفحص في التمييز بين امراض بطانة الرحم الموضعية والمنتشرة.

Triplex ultrasound findings of chronic leg ischemia with clinical correlation

Abstract

Background: Chronic leg ischemia commonly affects the arteries supplying the leg and mostly caused by atherosclerosis. Triplex ultrasound scanning is useful for delineating the anatomic site of disease in the lower limb in addition to the grading of the stenosis and evaluation of post-stenotic flow.
Objectives: To determine the extent of vasculopathy in patients with chronic leg ischemia by using triplex ultrasound scanning and match the sonographic findings with the clinical signs and symptoms.
Patients and methods: A cross sectional study from October 2009 to September 2010 was performed on 56 patients with clinical suspicion of chronic leg ischemia referring to ultrasound unit for triplex ultrasound examination of lower limbs at Al-Shaheed Ghazi Al-Hariri Teaching Hospital in Baghdad. Patients selected randomly and consisted of 19 women and 37 men. The age ranged between 20 and 90 year.
Results: The highest proportion of patients was within 60-69 year age group. Dorsalis pedis artery was the most frequently involved artery. Most of study sample revealed multi-level stenosis. Atherosclerosis was highest causative factor in chronic leg ischemia. The commonest clinical presentation of leg ischemia was intermittent claudication. Most of the patients show high-grade stenosis. Bilateral involvement is more than the unilateral, right side affected more than the left side. Diabetes mellitus considered the leading risk factor.
Conclusions: Triplex ultrasound of the lower extremities is useful to diagnose anatomic location and degree of stenosis of peripheral arterial disease, which is available and low cost.

Abstract

خلفية البحث: إفقار الساق المزمن هو حالة مرضية ناتجة عن تضييق أو انسداد في شرايين الأطراف السفلى نتيجة لتصلب الشرايين أو لأسباب أخرى وهذا بدوره يؤدي إلى أعراض وعلامات سريرية مختلفة. فحص الدوبلر له دور مهم في تحديد أماكن تضييق الشرايين ودرجة شدته وتقييم جريان الدم ما بعد التضييق .
أهداف البحث: تسعى الدراسة إلى كشف أهم العلامات في فحص الدوبلر لدى مرضى إفقار الساق المزمن وتحديد أماكن التضييق وشدته ومحاولة معرفة الأسباب المؤدية إلى التضييق إضافة إلى معرفة الأعراض السريرية للمرض وعلاقتها بنوع التضييق ومكانه وشدته.
المرضى وطرق العمل: هذا البحث عبارة عن دراسة وصفية اجري ما بين تشرين الاول من سنة 2009 إلى ايلول من سنة 2010 ، ولقد تم دراسة 56 مريضا يعانون من إعراض إفقار الساق المزمن عن طريق الاختيار العشوائي للمرضى من وحدة السونار في مستشفى الشهيد غازي الحريري للجراحات التخصصية في بغداد حيث تم إجراء فحص الدوبلر لكل المرضى الذين يتكونون من 19 امرأة و 37 رجلا ويتراوح أعمارهم ما بين 20 سنة إلى 90 سنة.
النتائج: لقد أظهرت النتائج بأن أكبر نسبة للمرضى كانت في الفئة العمرية 60-69 سنة وان معظم المرضى كانوا يعانون من تضيق متعدد المستويات وان اغلب المرضى أظهروا تضيق شرايين عالي الشدة وكان أكثر الشرايين إصابة هو شريان القدم الظهري. كما وجد أن تصلب الشرايين هو السبب الرئيس للمرض وأن معظم المرضى كانت شكواهم هي الألم المتناوب عند المشي. وكما وجدنا أن إصابة الطرفين أكثر عرضة من الطرف الواحد، وإصابة الطرف الأيمن أكثر من الأيسر. هنالك عدة عوامل خطورة للمرض ولكن وجدنا بأن داء السكري هو العامل الرئيس.
الاستنتاجات: تعتبر الدوبلر الفحص التصويري الابتدائي المختار لمرضى إفقار الساق المزمن لانه متوفر وبكلفة مناسبة ولا يحتاج إلى أي تداخل كما هو الحال في قسطرة الشرايين. وجد ان هنالك علاقة مهمة بين شدة العلامات والاعراض السريرية وشدة تضييق الشرايين وتعدد مستوياته.

Congenital lobar emphysema surgical versus conservative management

Abstract

Back ground: Congenital lobar Emphysema (CLE) is a rare yet serious condition which requires early detection and management, and usually present during infancy.
Objectives:
Patients and methods: This retrospective study aimed to review and evaluate methods adopted for the management of congenital lobar emphysema cases and it aimed to report and to document any observation which might need further research. The study retrospectively reviewed medical records of 53 cases whom were admitted to the department of thoracic surgery-medical city teaching complex over a period of 4 years (from June 2005 to June 2010). All of them had been diagnosed and managed as cases of congenital lobar emphysema based on their clinical picture and radiologic examination.
Results: The commonest age group affected were infants in the second month of life(40 patients) (75.47%), 10 patients (18.86%) within the next 3 months, 2 cases (03.77%)were over 6months old and only one case (01.88%) was over 1y. The number of male patients in this study was higher than the female patients (38 versus 15) with a male to female ratio was 2.53:1. The affected lobes were left upper lobe in 39 patients (73.60%), right middle lobe in 9 patients (16.98%), right upper lobe in 3 cases (5.66%), and right upper and middle lobes in 2 cases (3.77). Postoperative complication encountered in 6 patients; chest infection (2 patients), bronchopleural fistula and empyema (2 patients), wound infection (1 patient), and postoperative atelectasis (1 patient). Each treated accordingly. No intraoperative mortality was recorded, yet postoperative mortality happened in 3 cases, two cases due to respiratory failure and other one due to septicaemia. The significant finding in this review was that, 8 cases out of the 53, during the period of observation and conservative management, they eventually developed complete clinical improvement of their signs and symptoms and complete resolution of their radiologic findings. All other patients (45) required surgical resection, were lobectomy was the commonest type of surgical resection (39 patients), combined lobectomy and wedge resection performed in (6) patients.
Conclusions: A group of cases diagnosed with CLE can benefit from a period of observation and monitoring aiming for conservative management

Abstract

أنتفاخ الفص الرئوي الخلقي هي من الحالات النادرة ولكن الخطيرة والتي تتطلب تشخيص وعلاج مبكر وهي عادة ما تظهر نفسها بوقت مبكر بعد الولادة.
منهجية وطرائق البحث: هذه دراسة بأثر رجعي تهدف الى أعادة النظر والتقييم لمنهجية العلاج لحالات الأنتفاخ الفص الرئوي الخلقي التي تمت في قسم جراحة الصدر والأوعية الدموية في مدينة الطب التعليمية وتهدف أيضاً الى تثبيت وتوثيق المشاهدات الى تسترعي الأهتمام وتتطلب البحث. قامت الدراسة وبأثر رجعي بقرائة وتقييم الوثائق الطبيبة لخمسة وثلاثون حالة تمت أدخالها الى قسم جراحة الصدر والأوعية الدموية في مدينة الطب التعليمية من الفترة حزيران 2005 ولغاية حزيران 2010. تم تشخيص جميع الحالات على أنها أنتفاخ الفص الخلقي أعتماداً على الحالة السريرية والرقوق الشعاعية.
النتائج: أكثر الحالات كانت في الفئة العمرية التي في الشهر الثاني من العمر (40 مريض) (75.47%). عشرة مرضى (18.86%) في الأشهر الثلاثة التالية. حالتين (03.77%) وكانت أعمارهم أكثر من 6 شهر. في حين تم تسجيل حالة واحدة (01.88%) كان فيها عمر المريض أكثر من سنة واحدة. أتت نسبة الذكور أكثر من الأناث وبواقع 83 ذكر مقابل 15 أنثى وبنسبة 2.53:1. كان تويع نسبة الأصابة بين فصوص الرئتين كما يلي: الفص الأيسر العلوي 39 حالة، الفص الأيمن الوسطي 9 حالات، الفص الأيمن العلوي 3 حالات، وفي حالتين كان كلا الفص العلوي الأيمن والفص الأيمن الأوسط مصابان بالمرض. سجلت مضاعفات ما بعد العملية عند 6 ستة مرضى، أثنان التهاب الصدر، أثنان ناسور قصبي جنبي مع أنصباب قيحي في مريضين، التهاب الجرح في مريض واحد وأنكماش رئوي في مريض واحد. كل مريض عولج حسب حالته ولم تسجل وفاة أثناء العملية ولكن حدثت وفيات بعد العملية في ثلاث حالات، أثنان بسبب عجز تنفسي والآخر بسبب تسمم الدم الجرثومي.
الملاحظات المهمة في هذه الدراسة كانت في 8 ثمانية حالات من الحالات الثمانية والخمسين خلال فترة المراقبة والعلاج التحفظي أنهم تحسنوا بشكل كامل من الأعراض والعلامات السريرية والشعاعية.
كل المرضى الباقين وهم 45 خمسة واربعون مريض أحتاجوا الى أستئصال جراحي وكان رفع الفص الرئوي هو أكثر أنواع الأستئصال الجراحي حيث أجري لتسعة وثلاثون مريض. أما أسئصال فص رئوي مع أستئصال جزئي فقد أجري لستة مرضى.
الاستنتاج: وخلصت الدراسة الى أنه يمكن لبعض الحالات التي تم تشخيصها على أنها حالات أنتفاخ الفص الرئوي الخلقي أن تستفيد من فترة المراقبة لغرض معالجتها تحفظياً وبدون أجراء جراحي.

Background: Unstable pelvic ring fractures (PRF) are high energy trauma, commonly associated with systemic and soft tissues elements injures, the external fixation as an emergency and final treatment is one of the options in its management.
Objective: to evaluate the value and role of external fixation for unstable (PRF) and its clinical outcomes.
Patients and methods: twenty one patients with unstable P.R.F have been treated by external fixation , they were 16 men and 5 women ,their average age was 28 years (range ;18-60 years) , two thirds of the cases were due to road traffic accident , 85% were hemodynamicaly unstable , urological and neurological, anorectal injuries were the commonest associated injuries ,follow up period was 1.6 year range ( 1.00-6 years). Tile classification and Majeed outcome scale had been used.
Results: Excellent or good results obtained in 83% of Tile type B injuries , and 44% in type( C) P.R.F. Complications were noted in 42% in the form of skin problems , pin loosening, nerve injurey.
Conclusion; External fixation may be useful or even life saving in the acute resuscitation phase of the management of unstable pelvic fracture.

المقدمة:كسور حلقة الحوض إصابات ناشئه من صدمه خارجية فائقة الشدة .
مصحوبة بإصابات في محتويات الحوض الرخوة قد تكون خطيرة وقد تودي بالحياة .معالجة هذه الإصابات تتطلب السرعة .طريقة التثبيت الخارجي هي إحدى الاختيارات لمعالجة هذه المعضلة وقد تكون النهائية.
الغرض من الدراسة . تقييم قيمة ودور التثبيت الخارجي في معالجة كسور حلقة الحوض الغير مستقره .
المرضى وطريقة العمل : شملت الدراسة واحد وعشرين مريضا ستة عشر رجلا وخمسة نساء مصابين بكسور غير مستقرة في الحوض تتراوح أعمارهم بين 18_60 سنه وثلاثة أرباع منهم عانو من عدم استقرار الدورة الدموية بعد الإصابة بسبب النزيف . عولجو بطريقة التثبيت الخارجي. وتم متابعة حالتهم لفترة تتراوح بين 14 شهرا _37 شهرا .النتائج :أظهرت النتائج 83% منهم ممتازة إلى جيدة في الكسور الدرجة الثانية (ب) وبنسبة 44% في كسور الدرجة الثالثة (ج)وكان مقياس (مجيد)هو المعيار الذي صنفت النتائج بموجبه .
الاستنتاج: التثبيت الخارجي مفيد وقد ينقذ حياة المصابين بكسور الحوض وخاصة لنوع الكسر (ب)وبعض من كسور درجة (ج).

Combination therapy of Fludarabine and Cyclophosphamide(FC) Combination Regimen in advance stage of Chronic Lymphocytic Leukemia

Abstract

Background: the exposure of chronic lymphocytic leukemia (CLL) cells to fludarabine and cyclophosphamide resulted in an increased, synergistic cytotoxicity . DNA repair mechanisms in CLL cells, which are initiated in response to cyclophosphamide exposition, are inhibited by fludarabine. This observation was later translated into clinical trials evaluating the combination of fludarabine plus cyclophosphamide (FC) showed promising efficacy with response rates exceeding 90% in previously untreated and pretreated patient
Aim of this study: To assess the efficacy and safety of combination therapy of fludarabine plus cyclophosphamide in Iraqi adults patients with advance stage of chronic lymphocytic leukemia (CLL) in both previously treated and untreated patients and to assess the treatment free and overall survival in CLL patient who are receiving FC regimen therapy.
Patients and methods: Single arm study was done between February 2005 and fabruary2009.This study included 64 Iraqi patients aged between 39-77years old with advanced stage CLL .All patients received FC combination Therapy(fludarabine25mg/m2 plus cyclophosphamide 250mg/m2 for 3 days intravenously, repeated every 28 days). Treatment was administered for 4-6 courses. Forty eight(75%) patients were treatment naive(N) and 16(25%)were previously treated(Y) with alkylating agent.
Results: This combination chemotherapy resulted in 39.1% complete remission rate(CR) and 39.1%partial remission(PR) rate with overall response rated78.2%.The 2years median treatment-free survival was 90% with the median duration of response was 18months. Also there was a significant difference(p value <0.005) between different stage group(C&B) and degree of response, with better response rate in those with stage C than those with stage B, as the overall response rate was 88.3% and 65.5% respectively.
By common toxicity criteria (CTC) grading, the major toxicity(grade 3-4) in patients who treated by FC regimen were nausea and vomiting while the myelosuppression was prominent complication of grade 1&2 as leucopenia and neutropenia occur in 19%, 14% respectively however this is not increased the number of severe infections.
Conclusion: Fludarabine and cyclophosphamide combination regimen is an effective therapy for patients with advance CLL with high response and complete remission rate in those untreated and pretreated CLL patients, with good tolerability to this combination.

ان تعرض الخلايا السرطانية اللمفاوية لكلتا المادتين يزيد من فعالية قتل هذه الخلايا حيث أن عملية الإصلاح التي يمكن ان تحدث لمادة ال DNAفي هذه الخلايا كاستجابة لمادة السايكلو يتم تثبيطها بمادة الفلودارا مما يعطي استجابة قد تصل إلى أكثر من 90% من هولاء المرضى سواء تم علاجهم بعلاجات أخرى او لم تتم معالجتهم سابقا
الغرض :أن الغرض من هذه الدراسة هو تقييم كفاءة هذا البروتوكول العلاجي في الراحل المتقدمة لمرضى ابيضاض الدم اللمفاوي المزمن ونسبة البقاء على قيد الحياة وفترة عدم الحاجة فيها إلى علاج
في هذه الدراسة تم جمع المرضى من الفترة 2005 ولغاية 2009 ل64 مريض عراقي مصابين بابيضاض الدم اللمفاوي المزمن وبأعمار تتراوح مابين 39-77 سنة للمرضى لديهم مراحل متقدمة من المرض في 75% من هولاء المرضى كانت لديهم مرحلة C حسب تصنيف Binet بينما كانت مرحلة B في 25% من المرضى
النتائج :نتج عن هذا العلاج نسبة استجابة كاملة في 39% من المرضى واستجابة جزئية أخرى في 39% من المرضى أيضا وبالتالي كانت الاستجابة الكلية في 78% من المرضى نسبة البقاء على قيد الحياة وخلال فترة سنتين كانت 90% ومتوسط فترة الاستجابة 18 شهر دراسة التأثير الجانبي لهذا البروتوكول اظهرت حدوث حالات غثيان وقيء بصورة رئيسية فيما كان التأثير على نخاع العظم وتثبيطه بصوره ثانوية ومع ذلك لم تكن هنالك زيادة في الإصابة بالالتهابات الخمجية
الاستنتاج :ومن هذه الدراسة نستنتج ان هذا البروتوكول العلاجي فعال حتى في المراحل المتقدمة من المرض ذو استجابة عاليه سواء قد عولج المريض بعلاجات سابقه اوفي الحالات التي شخصت حديثا
