Clinical evaluation of selected Pharmacological Treatments used for Coronavirus (COVID-19) pandemic

Abstract

Background:Coronavirus is an enveloped RNA virus,from the genus Betacoronavirus, that could affectbirds, humans, and other mammals. The WHO has describedthe novel coronavirus disease as COVID-19(1). Objective:We have conducted this review to focus on the studies that assessed the treatment efficacy and safety of Coronavirus (COVID-19) and describe its relationshipwith the clinical outcomes of patients.Method:PubMed, was searched for studies on the clinical evaluation of selected currently used treatments for COVID-19. We included six studies about therapeutic activity of chloroquine/hydroxychloroquine, two case series about oseltamivir and three studies aboutlopinavir/ritonavirResults:Some of studies have been demonstrated and approved for a wider useofhydroxychloroquine for COVID-19, others showedthat there was insufficient evidence to recommendthe the routine use of thisdrug in patients admitted to the intensive care unit (ICU). Other treatments have insufficient evidence to recommend the use (lopinavir-Ritonavir or oseltamivir) for COVID-19 outside of research studies. Conclusion:In order to determine efficacy and safety of chloroquine/hydroxychloroquine for COVID-19, more randomized clinical trials are required. Abstract

Background: Native chest Computed Tomography (CT) is a quick, non-invasive and practical investigation andplays an important role in evaluation of Coronavirus Disease 2019.Objective:to describe the chest CT manifestations in patients with confirmed RT-PCR positive for coronavirus disease 2019 (COVID-19) in a case series from Baghdad,Iraq.Patients and Methods:The case seriesconsisted of 55 patients with laboratory confirmed COVID-19 for the period from 1stMarch through 15thApril 2020. Native chest CT was performed in the Department of Radiology, Medical City Teaching Complex (MCTH). The chest CT images were reviewed andanalyzedby the researchers.Results:The most frequent CT findings were ground-glass opacities (47.3%) andmixed ground glass andconsolidation (43.6%). Most lesions were multiple (67.2%), peripheral (56.3%) andbilateral (81.8%). Least common findings werepleural effusion (7.2%) andmediastinal lymphadenopathy (1.8%).Conclusion:The commonest chest CT findings of COVID-19 in Baghdad population were multiple, bilateral, peripheral ground glass opacity andconsolidation

Abstract

Background:Al-Najaf governorate, Iraq, has encounteredan outbreak and wide spread of the novel coronavirus disease 2019 (Covid-19).The epidemiologicalfeatures and clinical characteristics of (Covid-19) infection outbreak havebeenproclaimedbut a detailed clinical course and risk factors for mortality including medical comorbidities and severity of the illness at the time of presentation, have not been well described.Objective: To identify the potential riskfactors affecting the outcome of (Covid-19) infections.Methods:From February 24 to April 7, 2020, a case series study was done on 123 PCR-confirmed cases of (Covid-19) admitted to Al-Hakeem Hospital and Quarantine Center (AHQC) in Al-Najaf governorate, Iraq.Demographics, clinical and laboratory data were gathered from a local database at (AHQC). SPSS (Statistical Package for Social Sciences) was used for statistical analysis. The frequencies, percentages, means and standard deviations were used as descriptive statistics. The Chi square test or Yates correctedChi square test were used when appropriate, and (Pvalue ≤0.05) was regarded as significant. Results:Among the 123 patients, most infections occurred inside Iraq 96 (78%). The cohort included 67 (54.5%) females and 56 (45.5%) males, with a mean age of 32.6 ± 18.56 years. Most infections were mild or asymptomatic (72.3%). The most common symptoms were fever (74.8%), followed by cough (66.7%), headache (59.3%), and dyspnoea (28.5%). Most patients108 (87.8%) recovered while 10 (8.1%) patients improved clinically and awaiting the throat swab results,one patient (0.8%) remained in critical condition. Unfortunately,four patients died (3.3%). There is a significant association between medical comorbidities, severity of illness and patients’ outcome (Pvalue <0.001).Conclusions:The potential risk factors of medical comorbidities and severity of illness helped cliniciansand health workersto recognize patients with gloomyprognosis at an early stage and provided the rationale for a treatment strategy of infected patients. Repatriated cases, older age group, and male gender were associated with severe illness.

:تعرضت مدينة النجف الاشرف في العراق الى اصابات بالفايروس التاجي 2019(كوفيد 19). تم اعلانالصفات الوبائية والسريرية ل (كوفيد 19) لكن لم يتم توصيف تفاصيل المسار السريري وعوامل الخطورة للوفاة وبضمنها الاعتلالات الطبية المرافقة وشدة المرض في وقت تقديم المريض بصورة كاملة.هدف الدراسة:لتحديد عوامل الخطورة الكامنةالتي تؤثر على نتيجة الإصابة بالفايروس التاجي (كوفيد 19)طرق البحث:من 24شباط الى 7نيسان 2020تم اجراء دراسة حالات متسلسلة على 123حالة مشخصة بواسطة PCR(كوفيد 19) دخلت الى مستشفى الحكيم للعزل في محافظة النجف، العراق. تم جمع البيانات الديموغرافية والسريرية والمختبرية من قاعدة البيانات في مستشفى الحكيم العام. تم استخدام البرنامج الاحصائيSPSS في التحليل الاحصائي. استخدمت الاعداد، النسب المئوية، المتوسط والانحراف المعياري للأحصاء الوصفي، وتم استخدام اختبار مربع كاي للتحليل الاحصائي. تعتبر قيمةpاقل او يساوي 0.05 فرق معنوي.النتائج:ضمن ال 123مريض، معظم الإصابات حصلت داخل العراق 96(78%). المجموعة تتضمن 67(54.5%) اناث و56(45.5 )%ذكور، مع متوسط العمر 32.6 ±18.56سنة. معظم الإصابات كانت بدون اعراض او اعراض بسيطة (72.3%). اكثر الاعراض شيوعآ كانت ارتفاع درجة الحرارة (74.8%)، السعال (66.7%)، الصداع (59.3%)، ضيق النفس (28.5%). تم شفاء معظم المرضى 108 (87.8%) بينما تحسن 10(8.1%) سريريآ وينتظرون نتيجة مسحة الحنجرة. بقي مريض واحد (0.8%) في حالة حرجة. ولسوء الحظ، توفي أربعة من المرضى (3.3%). هنالك علاقة معنوية بين الاعتلالات المرضية وشدة المرض مع النتيجة النهائية (قيمة p<=0.001 .)الاستنتاجات:عوامل الخطورة الكامنة للاعتلالات المرضية وشدة المرض تساعد الأطباء لتحديد المرضى ذوي التنبؤ السيء في المرحلة الأولى وتعطي الأسباب للاستراتيجية العلاجية للمرضى المصابين . كان للإصابة خارج العراق، الكبار في السن، المرضى الذكور، علاقة مع شدة المرض.كلمات الدلالة:كوفيد-19، المصابين العائدين للوطن، الرصد الوبائي .

Abstract

Background:Soft tissue sarcomas (STS) are relatively rare tumors, representingless than 1% of adult malignancies based on report from American Cancer Society. The crude incidence rate in Iraq for soft tissue sarcomas was 219/ 100,000 populationsduring 2014 with a male: female ratio of 1.2:1, and the highest age peak falling between 30-34 years.The histological subtype and microscopic criteria are two parameters that influence the tumor grade which is the best predictor for aggressiveness.Objectives:A retrospective evaluation of soft tissue sarcomas, using immunohistochemical (IHC) study of Myosin, Desmin, Smooth muscle Actin (SMA) and Vimentin, aiming to reviewthe primary diagnosis and whether IHC assessment in soft tissue sarcoma is mandatory to reach a final interpretation.Patientsand methods:A total of 50 cases of soft tissue sarcomas were reviewedretrospectively from case archives of histopathology department/ Central Public Health LaboratoryandGhazi Al Hariri Hospital for Specialized Surgeries for the period (2011-2018). Hematoxylinand Eosin (H&E) stain and IHC staining of unstained sectionsfrom formalin-fixed, paraffin-embedded tissuewereperformed using SMA, Vimentin, Desmin and Myosin.The staining intensity was assessed based on the percentage of positive cell expression of the four IHC monoclonal antibodies.Results:The staining interpretation of the four monoclonal antibodies were confirmatory for the original diagnosisin 40% of cases,while 60% of the final diagnoses werere-evaluated and a different interpretation was givensupported bythe IHC findings for Desmin, Myosin, SMAand vimentin, under the clinical and radiological context.Conclusions: The co-expression of SMA and Desminhelped in the diagnosis of leiomyosarcoma (LMS).Myosin expression was helpful in the diagnosis of rhabdomyosarcoma (RMS)and alveolar soft part sarcoma(ASPS).SMAand Vimentinwere not specific for the diagnosis of malignant fibrous histiocytoma (MFH). By applying IHC markers, 60% of soft tissue sarcomas were found to be different from the preliminary diagnosiswhile 40% of cases were supportive of the original H&E diagnosis

:يعتبر غرن الانسجة الرخوة من الاورام النادرة الحدوث نسبيا حيث تشكل اقل من 1% من نسبة جميع السرطانات ،مع تنوع وتداخل كبيرين في الصفات النسيجية المرضية.كانت نسبة وقوع الورم في العراق حوالي 219 /100,000شخص للعام 2014وبنسبة ذكور لإناث تساوي 1.2 :1.ان الصبغات المناعية النسيجية الكيميائية تساعد بشكل كبير في التصنيف الدقيق لاورام الانسجة الرخوة. لقد اجريت هذه الدراسة لغرض تحديد اهمية وقيمة هذه التقنية في التوصل الى تشخيص دقيق لانواع مختلفة من غرن الانسجة الرخوة.الاهداف: اعادة تقييم التشخيص الاولي في50حالة من غرن الانسجة الرخوةمتضمنة نماذج الخزعات التشخيصية ونماذج استئصال الورم. تهدف الدراسة لتحديد اهمية اجراء الصبغات النسيجيةالمناعيةالكيميائية باستخدام الطرائق القياسية للحصول على افضل النتائج ودورها في التوصل الى تشخيص اكثر دقة.مادة الدراسة وطرق البحث:تم دراسة 50حالة مختارة لغرن الانسجة الرخوة استعاديا والتي تم الحصول عليها من مختبرات النسيج المرضي في مختبر الصحة العامة المركزي ومختبر مستشفى الجراحات التخصصيةفي بغدادللفترة الزمنية (2011-2018 .)تم استخراج قوالبالبارافين الشمعية وصبغها بصبغات الهيماتوكسيلين والايوسين (H&E) ،مايوسين للعضلات المخططة(Myosin) ،ديسمين(Desmin) ،أكتين العضلات الملساء(SMA)وفايمنتين (Vimentin).النتائج:اظهرت دراسة 50حالة من هذه الاورام باستخدام الصبغات المناعية الكيميائية تفاعلا ايجابيا اما موقعيا او منتشرا وبنسبة 71.4% للمايوسين،76% لأكتين العضلات الملساء ،16 %للديسمين و 84% للفايمنتين. مناقشة النتائج:ان التشخيص النهائي بعد اجراء فحوص الصبغات المناعية النسيجية الكيميائية قد تغير حيث كانت النتائج توكيدية للتشخيص الاولي في 40% من الحالات بينما 60% من الحالات اظهرت نتائج مساعدة في التوصل الى التشخيص بالتفريق عن حالات اخرى مشابهة شكليا او اعطت تشخيصا نهائيا محددا وواضحا ومختلفا عن التشخيص الاوليمع الاخذ بنظر الاعتبار نتائج الفحوص الشعاعية وموقع الورم للوصول للتشخيص النهائي.الاستنتاجوالتوصيات:التعبير المتصاحب لأكتين العضلات الملساء (SMA) وديسمين (Desmin) فيساعد على تمييز غرن العضلات الملساء بينما تعبير مايوسين العضلات المخططة ساعد على تشخيص غرن العضلات المخططة (RMS) وغرن الاجزاء الرخوة السنخية (ASPS.)استخدام أكتين العضلات الملساء وفايمنتين في تشخيص اورام الخلايا الليفية المنسجة الخبيثة (MFH) ولكن لايمكن الاعتماد عليها بصورة كاملة في تشخيص هذه الاورام لكونها حساسة ولكنها غير متخصصة لنوع معين من الاورام.من الممكن استخدام مجموعة اولية من معلمات الاورام النسيجية الكيميائية المناعية (مايوسين ديسمين ،اكتين العضلات الملساء وفايمنتين) كخطوة اساسيةاوليةفي تشخيص غرن الانسجة الرخوة بدقةوبالامكان الذهاب لاجراء صبغات نسيجية مناعية اخرى مثل (ٍS100بروتين ،مايوجينين ،سوكس 10 ،سي دي 34 ،سي دي 68و HHV8 )مفتاح كلمات البحث:غرن الانسجة الرخوة الخبيث ،الصبغات المناعية النسيجية الكيميائية ،تشخيص اورام الانسجة الرخوة

The Prevalence of Physical Disability in a Group of Elderly People Attending Primary Health Care Centers in Baghdad

Abstract

Background: The proportion of the elderlythroughout the world is continuously increasing, andbecoming a challenge, due tothe increasing number of the disabledand the rise of the national health burden.There is a greater need to look into their physical disability aspects, which are widely neglected.Objectives: To estimatethe prevalence of Activity of Daily Living dependency(ADL) among elderly people attendingprimary health care centers(PHCs) in Al Resafa health sector in Baghdad, and to explore the association between dependency and some socio-demographic factors.Method: A Descriptive cross-sectional study was carried out in all PHCs in Al Resafa health sector in Baghdad where 250 elderly clients were interviewed using a pretested questionnaire which covered someSocio-demographic factors and Barthal index scale(a 10 item instrument measuring functional independence in personal Activities of Daily Living).Results: Those interviewedwere 60 yearsor older, of whom those between 60 -69 years were 47.6%,and few were 80 years or older.There were slightly more femalesthan males (51.6% vs48.4%).Most of participants (97.6%)were living with their families, 82% were not currently employed, and 48% overweight.According to Barthal index scale score:78.0% were independent while ADL with assistance observed in 15.2% and those of ADL with severity were 6.8%. There was a significant association betweenincrease in age, female gender, low education and obesity and a decrease in ADL.Conclusion: Preserving functional capacity among the elderly is a great challenge to public health. Subjects’ categorization on the basis of Barthel index to correlate functional disability is of value for a better quality of life

ايد نسبة كبار السن في جميع أنحاء العالم باستمرار، ويصبح هذا التغير الديموغرافي تحديا في جميع أنحاء العالم، لأنه قد يؤدي إلى زيادة الاعاقةوزيادة العبء الصحي الوطني، وبالتالي هناك حاجة أكبر للنظر في جانب الإعاقة الجسدية، والتي تكون على خلاف ذلك مهملة. الأهداف: لتقدير مدى انتشارالاعاقة الجسديةبالاعتماد على الاستقلال الوظيفي في الأنشطة الشخصية للحياة اليومية لكبار السنالمراجعين في مراكز الرعاية الصحية الأولية في قطاع الرصافة، وايجاد العلاقةبين هذه الانشطةمع بعض العوامل الاجتماعية والديموغرافية.طريقة البحث:أجريتدراسةمقطعيةوصفيةفي جميع مراكز الرعاية الصحية الأولية في قطاع الرصافة وتمت مقابلة 250من كبار السن باستخدام استبيان اعد لهذا الغرض والذي غطى بعض العوامل الاجتماعية والديموغرافية بالاضافةالى مقياس مؤشر بارثل (المكون من 10عناصر تقيس الاستقلال الوظيفيفي الأنشطة الشخصية للحياة اليومية).النتائج: كان جميع المشاركين يبلغون من العمر 60نيبمهرامعأحوارتتنملكشو،قوفامواًماع60إلى 69اًماع47.6٪ من المشاركين، وكانت الإناث (روكذلانمًلايلقرثكأتانسملا51.6 ،٪48.4٪).كان معظم المشاركين 97.6٪ يعيشون مع عائلاتهم، وكان 82.0٪ منهم بدون عمل حاليا، وكان 48 ٪يعانون من زيادة الوزن. وفقا لدرجة مقياس مؤشر بارثال: 78.0٪ كان لديهم استقلال وظيفي في الأنشطة الشخصية للحياة اليومية في حين 15.2٪ كانوا بحاجةالى مساعدةوكان 6.8٪يعانون منشدةفيالأنشطة الشخصية للحياة اليومية.كان هناك ارتباط كبير بين الزيادة في العمر، الجنس الأنثوي، انخفاض مستوى التعليم والسمنة مع انخفاض الاستقلال الوظيفي في الأنشطة الشخصية للحياة اليومية.الاستنتاج:يمثل الحفاظ على القدرة الوظيفية للمسنين تحديا كبيرا للصحة العامة، وتصنيف القدرات على أساس مقياسمؤشر بارثل لربط العجز الوظيفي سيكون ذا قيمة لحياة أفضل

Abstract

Background: Antibiotics are medicines used to treat bacterial infections. Antibiotics resistance occurs when bacteria change in response to the inappropriate use of these medicines. Therefore, the world urgently needs to change the way that it prescribes and uses antibiotics.
Objective: To assess the knowledge, attitude and behavior towards antibiotics usage among non-medical students at the University of Fallujah, Iraq
Methods: A cross sectional study was conducted during the period from January to March 2019. Self-administered questionnaire comprised 25 questions divided into four parts, was used. The first part included demographic questions; the second, third and fourth parts were designed to assess the knowledge, attitudes and behavior of respondents toward antibiotics usage respectively.
Results: Females represented 54.8% of the subjects included in the study, and the age of subjects ranged between 19 and 24 years. About two thirds (64.1%) of participants believed that antibiotics work on most coughs and colds. In addition, 61.3% of the participants stated that “I will take antibiotic without prescription by a doctor”, while 55.6% of respondents preferred taking antibiotics with tea and only 39.1 % said that they took antibiotics with water.
Conclusion: Poor knowledge was detected among the non-medical students towards antibiotics usage. Health education programs might be needed among university students to improve their general knowledge and attitudes towards antibiotic

الخلفية: المضادات الحيوية هي أدوية تستخدم لعلاج العدوى البكتيرية. تحدث مقاومة المضادات الحيوية عندما تتغير البكتيريا استجابة للاستخدام غير المناسب لهذه الأدوية. لذلك، يحتاج العالم بشكل عاجل إلى تغيير الطريقة التي يصف بها ويستخدم المضادات الحيوية.

الهدف: تقييم المعرفة والمواقف والسلوك تجاه استخدام المضادات الحيوية بين الطلاب غير الطبيين في جامعة الفلوجة، العراق.

الطرق: تم إجراء دراسة مقطعية خلال الفترة من يناير إلى مارس 2019. تم استخدام استبيان ذاتي يتكون من 25 سؤالًا مقسمة إلى أربعة أجزاء. الجزء الأول تضمن أسئلة ديموغرافية؛ بينما تم تصميم الأجزاء الثانية والثالثة والرابعة لتقييم المعرفة والمواقف والسلوك للردود تجاه استخدام المضادات الحيوية على التوالي.

النتائج: شكلت الإناث 54.8% من المشاركين في الدراسة، وكان عمر المشاركين يتراوح بين 19 و24 عامًا. اعتقد حوالي ثلثي المشاركين (64.1%) أن المضادات الحيوية تعمل على معظم السعال والزكام. بالإضافة إلى ذلك، صرح 61.3% من المشاركين بأن \\"نيتي تناول المضاد الحيوي دون وصفة طبية من طبيب\\"، بينما فضّل 55.6% من المشاركين تناول المضادات الحيوية مع الشاي، وقال 39.1% فقط إنهم يتناولون المضادات الحيوية مع الماء.

الخاتمة: تم الكشف عن معرفة ضعيفة بين الطلاب غير الطبيين تجاه استخدام المضادات الحيوية. قد تكون هناك حاجة إلى برامج توعية صحية بين طلاب الجامعة لتحسين معرفتهم العامة ومواقفهم تجاه المضادات الحيوية.

Renal artery aneurysm (RAA) \"occursto focal dilatation of artery secondary to weakness of the arterial intima and media\",RAA is a rare condition withan incidence of 0.01 –1%.1Rupture RAAis considered as an extremely rare eventduring pregnancy. In this regard, the urologic and obstetric literatures are deficient. It is very difficult to diagnosearupture RAA during pregnancy,i.e. no pathognomonic presentation. During pregnancy and when rupture happened,the clinical presentation can be confused with those othercommon conditions e.g. rupture uterus or placenta abruption. Majority of the cases are incidentally discovered, even it is only identifiedafter autopsy

Abstract

Therearemyriads ofdietary and nutritional supplements available in the factual and virtual markets of the pharmaceutical industry[1, 2]. Consumersare avid totake those chemicals as complements to their regular dietto boost their health status, sexual endurance, physical performance,or even their cognitive abilities[3, 4]. Health authorities such as the World Health Organization, Food and Drug Administration in the United States, United Nations Office on Drugs and Crime, European Monitoring Centre for Drugs and Drug Addiction, and the Global Public Health Intelligence Networkshould regulate all chemicals[5]. Although there is an abundance of regulating and controlling agencies, an immense amount of unregulated or illegal chemicals, including supplements, exist either in the black market or virtually across the web [1, 3, 6-8]. Illegal tradingactivities require anonymity that flourishes in thedeveloping nations and electronically, via electronic commerce, over the darknet marketplace [6-8]. In 2008, “Satoshi Nakamoto” created the firstdecentralized digital currency system in the world, known as bitcoin [9]. The advent of cryptocurrencies was essential for untraceable illegal trade activities over the internet [10]
